Managing an SDAC Grant

  1. Initiate your FY21 Grant (July 1, 2020 - June 30, 2021): all grantees must complete and return the following:
    1. Signed Grant Agreement, please note, by signing the Grant Agreement, you agree to all of the terms and conditions outlined therein. If you need an additional copy of your Grant Agreement or the terms and conditionals of your agreement, please contact the SDAC Office at 605-773-3301
    2. W-9 form (We cannot except any other W-9 forms) Instructions for this form are available Instructions for W-9
    3. Arts Challenge Grantees must also submit the Projected Use of Funds Form. Instructions for Projected Use form are available.
  2. Request funds: grant payments are made in installments. Each request for payment is to be no more than the amount you will actually spend within a 60-day period. Requests may be made as needed to complete your activity, but they must fall within the correct fiscal year. To request funds from your FY21 Grant complete and return the following:
    1. Request for Payment Form
  3. To receive the final 10 percent of your grant, you must complete an Evaluation Form and Request for Payment within 30 days after the completion of the project or the end of the fiscal year (June 30th)—whichever comes first. Subsequent grants are dependent upon receipt of completed evaluation reports You must complete and return the following:
    1. Evaluation Form
    2. Request for Payment Form
